税務申告書

税務申告書には「tax return」を使います。（「tax report」、「tax declaration」より一般的です。）ですから、

法人税申告書　　corporate/corporation tax return
所得税申告書　　(individual) income tax return
消費税申告書　　(Japanese) consumption tax return

となります。例文を挙げると、

法人税申告書の提出期限は原則として決算日から2カ月以内である。
=> Corporation tax return must be generally filed within 2 months from the end of the accounting period.

「tax return」と相性のいい動詞（提出する）は「file」です。

昨日税務署に、所得税申告書を提出した。

I filed my income tax return with the tax office yesterday. 

～に提出するは「file ○○ to ～」ではなく「file ○○ with ～」です！
